Requirements
- Current/Expired License: Your physical license card
- Medical Certificate: From an LTO-accredited clinic
- PDC Certificate: Proof you passed the written/practical exams for the new code
- Comprehensive Driver's Education (CDE) Certificate: If it's your first renewal under the new system
