RENEWAL OF LICENSE
Requirements
- Current License: Your existing physical driver's license card
- Valid IDs: Original and photocopy of valid government-issued IDs (e.g., UMID, Passport).
- Proof of Birth: PSA-issued Birth Certificate (NSO is also accepted).
- Medical Certificate: From an LTO-accredited clinic, confirming you're fit to drive (often for adding codes/renewals).
Specific Scenarios
- Correcting Errors (Name, DOB, etc.): PSA Birth Certificate is key; sometimes NBI/Police Clearance might be needed if names differ significantly.
- Adding Restriction Codes: Requires passing the required tests (PDC/CDE) and a medical certificate, plus fees.
